PATIENT/PHYSICIAN ACCESS

Our Cardiac Health Program provides disease management therapies for patients who have been referred to us by their medical care providers. Members of our team are available Monday though Friday, from 7:30 am to 6 pm.

If you would like to participate in our program, please share this information with your physician and ask for a referral.

For Physicians

Physicians wishing to order our services should complete the preprinted clinic orders available in the Medical/Surgical, ICU and ED units.
