Cardiac Health Program

The Silverton Hospital Cardiac Health Program is designed to meet the special needs of adult patients and families who have had, or are at risk of, heart failure. We offer both inpatient and outpatient services.

Our program emphasizes individualized disease management with rigorous adherence to national heart failure guidelines. The program incorporates intensive education, ongoing monitoring and care by a specialized, multidisciplinary team.
